We require this information to verify that the image is legally usable on Wikipedia, and because most image licenses require giving credit to the image's creator.

After a quick look I would put the following banner if it was on the main space :

{{Multiple issues | autobiography = August 2009 | notable = August 2009 | unreferencedBLP = August 2009 | wikify = August 2009 | trivia = August 2009 }}

There are no secondary source (book, press coverage, …) to assess the notability of the subject. This is a critical issue. Once you've solved it (finding proofs you are notable), read WP:AUTO again and wait for someone else to write.
