Nelson Region is a redirect to Nelson, New Zealand. Each have categories. If you are asking why one is disambiguated, it's because Nelson is a disambiguation page because there are lots of people, places and things called that, but Nelson Region doesn't need disambiguation. There's a formal/legal distinction between the city and the region, but in practice one article covers them well. The demographics section gives figures for both the urban area and the region, because the population density is very different. The region category covers the rural areas as well as the city.
