Two young brothers live with their schizophrenic mother, who dies in the bathtub by electrocution. They are questioned about the turn of events, and the younger brother Anthony tells police that he saw his brother Tommy push the electric heater into the tub. The two are subsequently separated and put into foster care. Twenty years later, Anthony reappears in Tommy's life, and Tommy responds by shooting him. Goren and Eames are brought in to investigate the murder, and Rodgers tells them that the victim, Anthony (Patrick Arnheim), was dying from lymphoma. Meanwhile, over the years, Tommy (Sam Trammell) has been living a mysterious life, pretending that he is a graduate of Princeton University and acquiring several false identities. He rents a house on Nantucket under the name Tyler Chisholm, and the owner's daughter (Mireille Enos) takes a liking to him. The owner is suspicious and runs a background credit check on "Tyler." The credit check is reported to the detectives, as Tyler Chisholm was one of the names noted when interviewing people at the Princeton Eating Club. They travel to Nantucket, where they find Tommy and take him into custody. Back in New York, Eames questions the owner's daughter and asks if there was anything strange or out of the ordinary that she could recall. She tells Eames that she remembers Tommy throwing something into the water and staring at it. Goren then questions Tommy and reveals that Anthony did not return to bring back old memories and haunt Tommy, but to make amends because of his imminent death. Also stars Jenny Powers.

Devout Christian father, Paul Devildis (David Harbour), is aroused by his daughter Katherine's (Britt Robertson) performance in a low-cut period costume during a school production of "Cyrano de Bergerac". Later that night, he murders his liberal sister-in-law and her husband with a hammer at their home. Robert Goodman (Victor Verhaeghe) and cop Sgt. Whelan ([[Scott Sowers]]) are overly-zealous neighborhood watch leaders who post notices that released sex offender Kyle Jones (Darrin Malone) has moved in. The following day, Det. Streetman (Alison Becker) informs the Major Case Squad that his headless body was found floating in Maspeth Creek. Logan and Wheeler learn that Jones was 19 and the girl, Nancy Williams (Halley Wegryn Gross), who was 15 at the time admits the sex was consensual. He pleaded guilty to statutory rape under pressure from the ambitious ADA Terri Driver (Leslie Hope). Their case becomes complicated when a former detective, Clete Dixon (Skipp Sudduth), publicly promotes his theory that the murder is the work of a serial killer. Eventually, Logan and Wheeler focus on two local losers, Ricky Moss (David Call) and Lou Olsen (Lucas Papaelias). They push Ricky to confess that he carried out the murder and decapitation to impress a neighborhood girl, Brenda Lally (Dreama Walker), although he admits that he "didn't even enjoy it". Jonathan Cake briefly appears as Colin Ledger, Det. Wheeler's husband.

Father Chris Shea (Denis O'Hare), a friend of Logan's former partner Lennie Briscoe, tells Logan a dying man has confessed to three murders - Peter Bottner (Alexander Cole), his pregnant wife Shannon (Heather Dilly) and their unborn child. Logan and Wheeler look into the 16-year-old triple homicide case which resulted in the conviction of Randy Nichols (Chris McKinney) who still has eight years left on his sentence. The detectives find his prosecutor was a young ADA, Terri Driver (Leslie Hope) who manipulated evidence to gain a quick conviction for personal gain. She is now running for Attorney General and threatens both Logan's and Wheeler's careers while she arranges for Father Shea to be stabbed during a prison visit. Logan pushes on regardless and discovers that the murderer was Bottner's friend and business partner Marty Swenders (Matthew Detmer) using the name Dylan Farlis who swindled Bottner's father Alan's (James Murtaugh) company of millions. Logan pressures Driver to reluctantly reopen the case and quash Nichols's conviction, however she has Wheeler's fiancée arrested by the FBI for financial crimes and makes Logan's position untenable. In the final scene, the recovering Father Shea cautions Logan to control his anger think more about his own welfare.

When he goes to visit Frank he finds a police investigation after his brother fell from his window. Dr. Elizabeth Rodgers concludes that he was murdered after being injected with a paralyzing drug, and evidence points to the involvement of Nicole Wallace (Olivia d'Abo). Goren is then called to the hospital bedside of Dr. Declan Gage (John Glover), who has been poisoned with a paralytic drug and begins a hunt to find Wallace. He and Eames visit Gwen Chapel who is dying from cancer and then following a lead, they go Massachusetts where they find a heart in a box which Dr. Elizabeth Rodgers determines is that of Wallace after DNA testing but Goren refuses to believe it. This causes the Captain starts an investigation of Goren which also reveals that the serial killer Mark Ford Brady is his father.
