Practice Transformation is a change management process specific to health care delivery system reform introducing quality improvement measurement and reporting.[1] Practice transformation is simultaneously comprised of community-level action supporting both physical, mental, and behavioral health coordination across care settings and inclusive of broad national health systems reform efforts infusing quality measurement, health outcomes reporting, and informing responsive health care systems evolution.[2],[3]
