This template can be placed either in an article or on its talk page. If placed in an article it looks like this:

This article is about a topic whose name is originally rendered in the Hebrew script; however the article does not have that version of its name in the article's lead paragraph. Anyone who is knowledgeable enough with the original language is invited to assist in adding the Hebrew script.

And if placed on a talk page it looks like this:
