Responsibility assignment matrix This matrix is based on RACI-VS concept, that mean who's Responsible, Accountable, Consulted, Informed, Verify and Sign. You must draw a table, on each row you have Activity and on each column one of initials concerned people. After that you complete your table with one of characters : R/A/C/I/V/S
