The documents I've found are dated 1993, still there's also evidence of 2001 as the date. I've edited adding both. The 1993 version seems a 'manuscript' from the University of Michigan. Some evidences:

Requested move
I have requested a move, hopefully to either Keyboard-Level Model (the term used in my Software Usability course textbook) or Keystroke-Level Model (the term used in this article). The other term should redirect to the article. I disagree with the use of acronyms as the sole title of the article. ThomasOwens (talk) 12:52, 23 March 2009 (UTC)
