1. Suspected sockpuppets - enter each of the other users or IPs you believe may be misusing accounts together, one on each line, like this:

2. Evidence - enter your evidence. This is the basis of your case. It should make clear that a user is, likely to be abusing multiple accounts, based upon the evidence that you present. Also, ensure that you provide diffs that justify why the persons are a) the same and b) disruptive. This should be "dense", that is, solid with evidence, and information needed to judge the matter, and avoid "general discussion" that is not to the point.
3. Comments by accused and by other parties - Exactly the same, your evidence (if any) that you wish considered to show the case is unjustified. If multiple people use one section, please keep "like with like" - one persons comments, then the next. Do not thread or reply to other people's sections, these will be moved.
4. Clerk, and Conclusions - leave these blank. The clerks who patrol these pages will use this section to add their notes. If you wish to reply, do so briefly and to the point, otherwise consider putting longer replies in your own section.
5. Do NOT enter an edit summary, even if asked to - When you click save, you may be asked to enter an edit summary, or told you have to. The template already includes this.
