His name Muhammad Ashfaq is recorded in official report of the 1968 Summer Olympics (Volume III, pp.707-7015). Though spellings of his first name in the said report are Mohammad, these have now to be written as Muhammad because of Ministry of Religious Affairs (Govt of Pakistan)'s instructions about how to...). Muhammad Ashfaq (November 11, 1946 – July 3, 2005) was a Pakistani field hockey player. He won a gold medal at the 1968 Summer Olympics in Mexico City.[1]

Death

He died on 3 July 2005 in Rawalpindi, Pakistan, and was buried in local Alif Shah graveyard.
